The Behavioral Risk Factor Surveillance System, an annual survey conducted by the CDC should have some of the statistics you are looking for. You can look at data at the city/county level for tobacco use here: http://apps.nccd.cdc.gov/brfss-smart/SelMMSAPrevData.asp
